Simply complete the Term Life Insurance section on your application.*** Blue Preferred Term LifeT insurance is available with the following health policies: KeyCare Flexible Choce, KeyCare HealthSmart, KeyCare HealthSmart with Enhanced Drug Benefit, Lumenos HSA, Lumenos HIA and Lumenos HIA Plus.  

***Please note that if you complete the Term Life Insurance section and there is a domestic partner for whom coverage is being requested, he/she must cosign the application. If approved, you and your domestic partner will be set up on two separate policies, each including the health and life coverage selected on the application.  

You can manage all of your Anthem insurance coverage with just one bill.  

There's only one bill for health and term life coverage and you can even select coverage for all of your family members. 

The Blue Preferred Term LifeT insurance policy can also be renewed, but is a separate policy from your health insurance policy. Anthem Life Insurance Company, an affiliate of Anthem Blue Cross and Blue Shield, insures the Blue Preferred Term Life T insurance policy.

Please note that in order for your family to enroll in the Term Life policy, all family members must meet certain health underwriting guidelines for the health insurance policy .

*$15,000 policy is available to dependent children only, with a maximum of three dependent children. More than three children can be added to the plan, but no additional premium will be charged. Spouses are not eligible for the $15,000 plan and must select the same plan as the subscriber.
** Coverage is not available for children less than one (1) year of age.
Children under age 1 will be automatically added to the policy on the policy anniversary after they turn 1.
+ The Blue Preferred Term Life T coverage option $75k for applicant /$75k for spouse/$15k per dependent child is available only with Lumenos HSA, Lumenos HIA, or Lumenos HIA Plus.
Acceptance into an Anthem Life policy is contingent upon your acceptance into an Anthem underwritten health plan. Eligibility for Blue Preferred Term Life TM coverage is dependent upon the underwritten approval of all applicants for the health policy applied for, and is not available for those applicants listed as HIPAA Eligible Individuals who receive a Level 4 underwriting decision. Blue Preferred Term Life TM insurance is available only with Anthem plans Individual KeyCare Flexible Choice, KeyCare HealthSmart, KeyCare HealthSmart with Enhanced Drug Benefit, Lumenos HSA, Lumenos HIA, and Lumenos HIA Plus. This term life insurance coverage cannot be purchased if intended to replace another life or annuity policy.
